Working together, you can have that smile you've always wanted.

Your First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • X-rays if you have any
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

Whether you need a second opinion, you're a new patient, or you want to learn about cosmetic dentistry, Dr. Collins will be happy to provide a complimentary consultation for you.

For Your Comfort

If you’re one of the millions of Americans who has stress or anxiety about visiting the dentist, our team will go above and beyond to ensure a comfortable experience. In addition to listening to your concerns, answering your questions, and earning your confidence, we can provide nitrous oxide to enhance your comfort during clinical procedures. Many people know nitrous oxide as laughing gas. After you’re comfortably seated in the dental chair, you’ll inhale the gas (a nitrous oxide and oxygen mixture) through a face mask. You will not recognize pain, so Dr. Collins can perform your dental work while you stay completely comfortable. The effects of nitrous oxide subside once we remove your mask, so if nitrous is the only anesthesia you receive, you will be capable of driving yourself home following the visit and can continue your daily activities as normal.
